Solicitud del paciente
If you are our patient and would like to request your medical records, please click on the "Begin" link below to complete your request.
You will be asked to provide a valid email address and a copy of your ID as part of the process.
Solicitud de terceros
If you are a healthcare provider, law office or if you are requesting medical records of someone other than yourself and are not the legal guardian, please click on the "Begin" link below to complete your request.
Please do not upload subpoenas or court order. All subpoenas and court orders must be submitted to the facility directly.
Note: You may be contacted to provide additional documentation, such as a birth certificate, power of attorney, etc. You will receive an update or response from Sharecare within 3 business days.
